Add Visa Pages to US Passport
You can add extra pages to your passport if:
- Your most recent passport is still valid
- You can submit your most recent passport
- Your passport is in a good condition (not mutilated/damaged)
- You are not making any changes in your identifying information of your passport
Please don't hold on to your application and other paperwork when signed and dated. Send them to us ASAP. Once the documents are dated they must be in our office within four calendar days, and submitted to the Government on the fifth day; applications will be rejected if not submitted within five calendar days from signed date.
If You have less than 2 to 4 Visa pages remaining in your passport book- FOLLOW the instructions on this page
If you travel extensively and are applying for a new passport- FOLLOW the instructions for US Passport Renewal (Form DS-82) if your passport is less than 15 yeas old.
FOLLOW the instructions for First Time Passport for Adults (Form DS-11) if your passport is more than 15 yeas old. - PLUS A signed, written statement addressed to US Department of State requesting a 48 page passport.
For us to process your request properly and on time you MUST send International Passport and Visa Service Request Form with the following documents:
1 - International Passport & Visa Authorization Letter International Passport & Visa Authorization Letter, completed and signed.
2 - Most Recent US Passport 
3 - Application for Additional Visa Pages Form DS-4085, completed.Note: Application is not compatible with Safari web browser.
4 - Proof of Travel Copy of your travel itinerary OR a business letter on your company’s letter.You have to provide proof of travel overseas in the next 2 weeks (if no visa is required) and in the next 4 weeks (if a visa is required).
