Effective September 26th 2005 passport agencies are no longer amending a passport for reasons such as change of name through court, and change of name by marriage - a new passport is issued
For us to process your request properly and on time you MUST send International Passport and Visa Service Request Form with the following documents:
Please don't hold on to your application and other paperwork when signed and dated. Send them to us ASAP. Once the documents are dated they must be in our office within four calendar days, and submitted to the Government on the fifth day; applications will be rejected if not submitted within five calendar days from signed date.
Your passport was issued less than 1 year ago and your name has been legally changed due to marriage or court order- FOLLOW the instructions on this page
Your passport was issued more than 1 year ago and your name has been legally changed due to marriage or court order- FOLLOW the instructions for US Passport Renewal (Form DS-82), Application for a U.S. Passport by Mail.
- PLUS A certified copy or a complete and unaltered photocopy of your marriage certificate or court order showing a seal and official signature.
Your name has been legally changed due to marriage or court order (regardless of the age of your passport) BUT you cannot legally document your name change (through court order or marriage- FOLLOW the instructions for First Time Passport for Adults (Form DS-11), Application for a U.S. Passport.
- PLUS at least 3 public records showing your date and place of birth that show exclusive use of your assumed name for at least 5 years

1 - International Passport & Visa Authorization Letter International Passport & Visa Authorization Letter, completed and signed.
2 - Your current, valid US Passport 
3 - Application for a U.S. Passport: Name Change, Data Correction, and Limited Passport Book Replacement Form DS-5504, completed.Note: Application is not compatible with Safari web browser.
4 - Marriage Certificate or Court Order A certified copy or a complete and unaltered photocopy of your marriage certificate or court order showing a seal and official signature. 
6 - Proof of Travel Copy of your travel itinerary OR a business letter on your company’s letter.You have to provide proof of travel overseas in the next 2 weeks (if no visa is required) and in the next 4 weeks (if a visa is required).
