To obtain a U.S. Passport for a child, the following must apply:

  • Applicant is a U.S. citizen under 16 years old
  • and one of the following:

  • Applicant has never had a U.S. passport issued before, OR;
  • Applicant’s passport has been lost, stolen or damaged, OR;
  • Applicant's passport has or will expire soon

Children's passports can not be "renewed." Each passport issued to a minor under 16 years will be considered a "new" passport and must go through full verification process.

Complete DS-11: Application for a U.S. Passport using the online form filler.

Go to Application Form

Do not sign the application until instructed to do so by the Acceptance Agent.

  • Print two (2) copies of your completed DS-11 form single sided.
  • Improperly completed applications will delay the processing of your passport.
  • Hand written applications will not be accepted.

Original certified birth certificate or naturalization certificate or previous passport.

Applicant's parent(s) or legal guardian(s) must have a driver's license, state-issued ID or valid passport.

In order to apply for a passport, parent(s) or legal guardian(s) listed on the child’s birth certificate or legal decree must be present at the Acceptance Agency (post office, etc.).

Additional Info
  • If one parent or legal guardian cannot be present, that parent may provide a notarized Form DS-3053 authorizing the child to apply for a passport. This is meant to be utilized for emergency situations, not convenience.

  • If only one parent is listed on the birth certificate or a legal decree provides for full custody, the second parent or guardian requirement is waived.

  • If the name on the ID of the parent/guardian is different than what is on the child’s birth certificate, then evidence of legal name change is required.

Copy of your round-trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent.

  • The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information.
  • If you have not purchased your tickets, please call us (310-274-2020) to further assist you.

Please print two (2) signed Letter of Authorization forms. This allows us to process your passport on your behalf.

Check the 1st and 2nd boxes on the form. This allows us to expedite the processing your passport application.

  • Both must have original signature, blue or black ink.
  • Put one original signed letter inside the sealed envelope, and attach the other original signed letter to the outside of the sealed envelope.

As part of this process, you must provide two personal payments to complete your application.

  • Government fee

    Personal check, cashier’s check or money order of $140

    Payable to U.S. Department of State

  • Execution fee

    A separate execution fee of $35 will be charged by the Acceptance Agent (post office, etc.)

    Payable by cash or credit card, in most cases.

  • Please make copies of all your supporting documents (proof of identity, passport photo etc.). for your records BEFORE visiting our office. Copies made in-office carry a surcharge.

    Out-of-state orders – Please send copies of all your supporting documents (proof of identity, passport photo etc.). BEFORE making your appointment with a Passport Acceptance Agent to review everything for accuracy.

Service Fees – General Information
  • Applications begin processing the business day after your documents are completed and ready to submit.
  • If shipping your documents to us, processing times do not include return shipping estimates.
  • Processing times and fees are subject to change without notice.
  • Consulate fees may require a U.S. Postal Money Order. A $15 fee applies if IPV obtains a money order on your behalf. If providing your own U.S. Postal Money Order for your visa application, the fee will be waived.

How It Works

Gather all your documents and take them to an Acceptance Agent.

  • An Acceptance Agent is a government employee who will verify your identity or the identity of the applicant.
  • Acceptance agents are at Post Offices and Courthouses. We can help you book an appointment at a post office.
  • You must indicate that you are using a “Hand Carry” service.
  • The Acceptance Agent will have you sign the application in front of them.
  • The Acceptance Agent will seal all documents in an envelope and return the sealed envelope to you.
  • Drop off the sealed envelope at our office or send it to International Passport & Visa for expedited processing.

Sealed Envelope

When appearing at a Passport Acceptance Agent you will be submitting all of your documents, the government fee and photos.

Once you have completed the application and gathered all of the supporting requirements you and your child will then take everything to a Passport Acceptance Agent, who will review the document requirements (proof of citizenship, etc.) and witness you sign the application form.
