To replace a lost or stolen U.S. Passport, the following must apply:

  • You have lost a valid U.S. passport.
  • or

  • Your valid U.S. passport has been stolen.

Reporting a lost or stolen passport will place it immediately into the Lost and Stolen Passport System. As soon as you CREATE the form when you complete the application online, the passport being reported is invalidated. The passport cannot be used for travel and cannot be reinstated or reactivated. If the passport is found at a later time, it must be reported as found and submitted for cancellation.

Original certified birth certificate or naturalization certificate or previous passport.

You must provide 2 photocopies (front & back) of your valid driver's license or state ID card when applying for a U.S. passport.

Any of the following will serve as primary proof of identity:

  • Previous U.S. passport (mutilated, altered, or damaged passports are not acceptable)
  • Naturalization Certificate
  • Driver's License
  • Government ID: city, state or federal
  • Military ID: military and dependents

Two (2) passport style photos.

  • 2" x 2" size
  • white background
  • taken within 6 months

Your photo should not be attached to the form.

Copy of your round-trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent.

  • The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information.
  • If you have not purchased your tickets, please call us (310-274-2020) to further assist you.

Please print two (2) signed Letter of Authorization forms. This allows us to process your passport on your behalf.

Check the 1st and 2nd boxes on the form. This allows us to expedite the processing your passport application.

  • Both must have original signature, blue or black ink.
  • Put one original signed letter inside the sealed envelope, and attach the other original signed letter to the outside of the sealed envelope.

As part of this process, you must provide two personal payments to complete your application.

  • Government fee

    Personal check, cashier’s check or money order of $170

    Payable to U.S. Department of State

  • Execution fee

    A separate execution fee of $35 will be charged by the Acceptance Agent (post office, etc.)

    Payable by cash or credit card, in most cases.

Applicant must submit one completed Statement Regarding a Lost or Stolen U.S. Passport (Form DS-64).

It is not uncommon for applicants to not know their previous passport details. In this case these fields may be left blank.

When explaining how the passport went missing, be as detailed as possible. If unknown, applicant should indicate the extent to which they have searched including specific locations such as house, cars, office, etc.

Do not sign the application until instructed to do so by the Acceptance Agent.

  • Print two (2) copies of your completed DS-11 form single sided.
  • Improperly completed applications will delay the processing of your passport.
  • Hand written applications will not be accepted.

  • Please make copies of all your supporting documents (proof of identity, passport photo etc.). for your records BEFORE visiting our office. Copies made in-office carry a surcharge.

Service Fees – General Information
  • Applications begin processing the business day after your documents are completed and ready to submit.
  • If shipping your documents to us, processing times do not include return shipping estimates.
  • Processing times and fees are subject to change without notice.
  • Consulate fees may require a U.S. Postal Money Order. A $15 fee applies if IPV obtains a money order on your behalf. If providing your own U.S. Postal Money Order for your visa application, the fee will be waived.

How It Works

Gather all your documents and take them to an Acceptance Agent.

  • An Acceptance Agent is a government employee who will verify your identity or the identity of the applicant.
  • Acceptance agents are at Post Offices and Courthouses. We can help you book an appointment at a post office.
  • You must indicate that you are using a “Hand Carry” service.
  • The Acceptance Agent will have you sign the application in front of them.
  • The Acceptance Agent will seal all documents in an envelope and return the sealed envelope to you.
  • Drop off the sealed envelope at our office or send it to International Passport & Visa for expedited processing.

Sealed Envelope

When appearing at a Passport Acceptance Agent you will be submitting all of your documents, the government fee and photos.

Once you have completed the application and gathered all of the supporting requirements you and your child will then take everything to a Passport Acceptance Agent, who will review the document requirements (proof of citizenship, etc.) and witness you sign the application form.
